Need help with form page with date Search criteria

Hi Everyone,
I am trying to create the form page with search criteria of date and date range. i need help to create a form page. i am new to access and does not know much of access. i have attached the access db which has Table uploaded. and word page , how my boss wants the search page to look like.
can anyone help please ?
