Form Design

I've created some forms but don't understand how to keep one portion of it while the other portion changes. Here's what I'm trying to say.

At the top of my form are fields such as: date, model: user: etc. While beneath these fields are criteria that needs to be audited and each of these fields has a check box.
What I'd like to do, if possible, is keep the upper fields (date, model, etc) and have only the lower portion change when a new record is started.

I've attached a couple of snap shots that show my form both in design and form format.

I'm sure I haven't explained myself very well but I'll try to answer any questions you may have. I'm not very good with all of this however, in the past I have created a database or two with the help of you folks. I'm hoping for more of the same with this one.
